Dear Fellow Member,

As we all know it is sometimes difficult to line up speakers for our monthly chapter educational meeting. For the most part the job is given to the educational chairman, which may or may not have the time or the personal contacts to acquire a qualified speaker.

CARSES has put together the following speaker release form to help solve this problem. When filled out and returned, the information will be posted on the CARSES web site where all chapters in Region 14 & 15 will have access. We urge you to present this form to your speaker and mail or fax the original to the CARSES web site director.

Note: (the form must be signed by the speaker.)
